How to make a request
Requests for records pursuant to the Act can be submitted to the NNSA Albuquerque Complex in writing at the following address:

In your request, please advise you are requesting the records pursuant to the Privacy Act of 1974 and describe the records being sought as clearly as possible and state whether you are seeking access or requesting an amendment. You may request to review, in person, the records being sought. If you are a former employee, for example, providing the dates of your employment would be helpful in locating records. Other information that would be helpful in locating records would be your full name, any other names you might have used, your birth date, your social security number, and home address. Please provide a phone number where you can be reached during the day in case there are questions. Don’t forget to sign your request. Also, you will need to establish your identity. This can be accomplished by seeking copies of or access or requesting an amendment to records.
